Required Information

We need the following information before we can decide if we will accept a student.

• A psychological evaluation, including assessment of intelligence and emotional status.
• An educational evaluation, including assessment of achievement, educational needs, and school adjustment.
• Social case history information.
• A report of physical examination and medical history, to include a drug use profile which shall include:
• A history of prescription and non- prescription drugs being taken at the time of admission and for the previous six months
• Ineffective chemotherapy.
• A statement of your expectations.

Sometimes, more information is needed before we can make a decision about acceptance. We may ask you to provide this information to us. It may include:

• A report of neurological evaluation.
• A report of psychiatric evaluation.
• A report of Juvenile Court contacts.
• A report of vision, hearing, and speech screening.
