Placement Agreement & Permission Forms

Trusting your child to someone else's care is a difficult decision. Taking care of your child is a responsibility we take seriously. A written Placement Agreement outlines our mutual responsibilities for your child's care.

We will keep you informed of your child's care and his activities. Some of these activities require very specific parental permission, such as participation in religious activities and athletics, and releases for media. Some involve the safety and welfare of your student, such as permission to secure emergency medical treatment and permission to search. Each of these permission forms will be discussed thoroughly with you on acceptance of your student.
