DIAGNOSTIC SERVICES

A comprehensive evaluation provided during a 15-week placement to provide a thorough understanding of an adolescent. This service occurs within a highly structured therapeutic environment that teaches accountability and responsibility.

Assessment includes:

  • Psychological
  • Academic (Adolescent may earn 2 credits during stay)
  • Medical
  • Social functioning
  • Family relationships
  • Career interests

Planned Length of Stay

First 8 weeks: Completion of assessment and written report.

9th week: Feedback and Planning meeting is conducted with all involved parties to finalize Discharge Plan.

10th - 15th weeks: Devoted to providing case coordination services, and transition to the next most appropriate, least restrictive level of care.

Transitional Services

Based on the assessment information and recommendations,
detailed planning will occur to connect the adolescent
with the next level of care or networked supports
decided upon and prepare him to successfully
transition.


GOAL-BASED SERVICES

Goal-Based Services are appropriate for adolescents who must master specific objectives in order to transition successfully back to their community.

  • Relatively “short” placement period.
  • Residential placement is initiated on the basis of specific needs demonstrated by the adolescent and family.
  • Goals are developed based on the needs.
  • Achievement of those goals is the criteria for discharge.
  • High to moderate support will be required in the community to maintain and promote progress.

Developmental Program

The adolescent progresses through the developmental program until demonstrating specific outcomes indicative of the goal achievement set at admission.

Family Services

Work with the family begins upon admission and continues
throughout placement as an integral part of treatment.

Transitional Services

Discharge planning begins at the time of admission. The agreed upon plan includes specific transitional activities required for continued success after discharge.

General Services

All services available at Timber Ridge School as a licensed Residential Treatment Facility are available to adolescents receiving Goal-Based Services.


IN-HOME SKILL DEVELOPMENT SERVICES*

In-Home Skill Development Services helps family members understand their role and develops necessary interpersonal and parenting skills essential to healthy family functioning.

Intensive In-Home Services

  • Provides assessment and family counseling to clients at risk of out-of-home placement.
  • Improves the adolescent’s behavior and structures the home environment.
  • Teaches behavior management techniques.
  • Builds productive and positive adolescent / parent relationships.

Supportive In-Home Services

  • Provides socially necessary services to clients demonstrating a general dysfunction within the family.
  • Provides supportive counseling to a family of an adolescent returning from out-of-home placements.
  • Focuses on reinforcing the positive changes that occurred during the out-of-home placement for the adolescent and family.
  • Transitional work with the out-of-home provider begins prior to discharge.
  • The Family Team Planning goals focus on transitional objectives.
  • Family Intervention Services
  • Provides in-home counseling services.
  • Increases effective communication, diffusing crisis, and stabilization of the home environment.
  • Develops and implements an after-care treatment plan to prevent future out-of-home placements.

STABILIZATION SERVICES

Stabilization Services prepare the adolescent for “stepdown” to a lesser restrictive in-community setting where continuing services will be provided.

Treatment Time Frame: 3 to 6 months

Goals for Stabilization and Reintegration

  • Successfully complete three developmental levels.
  • Help the adolescent identify and define community norms.
  • Establish clear expectations and rules within the home environment.
  • Establish the necessary support systems within the community.
  • Work intensely with the adolescent and adult / family to which he will be discharged.

Transitional Services

Upon completion of Level 2 and initiation of Level 3, the adolescent will be regularly involved in supported, transitional activities moving toward discharge.
