Initial Individualized Service Plan (ISP)


We begin to develop an initial Individualized Service Plan (ISP) when we first review the referral material. At this time we determine the current level of functioning of the student, the current needs presented by the student, and any additional information we need during the personal interview.

During the interview, we will share this information with you. We will ask you for your views and concerns and we will talk about how we plan to meet these needs. Dr. Ken Weigand, our Clinical Psychologist, will take this information and develop a draft plan. When your student is admitted, we will review and amend this plan.
All of us will sign the plan and each of us will have a copy of the plan for reference. As the student progresses, we will send you reports of his achievements of the goals in this plan.
