Required Information & Application Process

The following documents are required:
- Completed Application for Admission
- Psychological evaluation, including assessment of intelligence and emotional status. Authorization for Release of Psychological Information
- Academic history and educational evaluation, including assessment of achievement, educational needs, and school adjustment; current report card, transcript, and results of any Standards of Learning (SOL) testing, current Individual Education Plan (IEP), and minutes from the most recent eligibility meeting if the student is eligible for Special Education Services. Authorization for Release of Academic Information
- Social case history information.
- Report of physical examination and medical history, including a drug use profile with a history of prescription and non- prescription drugs taken at the time of admission and for the previous six months, as well as any history of ineffective psychotropic medications.
- Family Participation Agreement
The following reports might also be required:
- Psychiatric evaluation. Authorization for Release of Psychiatric Information
- Neurological evaluation.
- Juvenile Court contacts.
- Vision, hearing, and speech screening.
After acceptance, the Rotz Pharmacy Information Form must be completed one week prior to Timber Ridge School arrival for students who are currently prescribed medications of any type.
